AX-5 REBUILD HELP

I've been driving for a week on a newly rebuilt (by a buddy) ax-5 transmission with no problems. Then, suddenly, when shifting to Reverse, I get locked out of all the gears except 5th and reverse. After pulling the shifter tower out I can see that the reverse lockout lever has positioned itself above the shifter fork. With a screw driver I depresed the lockout lever and the fork was then sliped up into place. I've had this problem twice now and although my buddy is a hella John Deere mechanic, this is his first ax-5. What could have gone wrong in the rebuild that causes the clearance that allows the lockout lever to pop up so high? A possible contributing factor is the pivot ball slots that are cut into the side of the ball are worn causing a bit of a sloppy fit into the top transmission plate that it sits in.
 

I should add that the pivot ball that I'm referring to is the one on the bottom of the stick shift.
